Register for any SWIP Training Workshop with this form.  Within two weeks of submitting this registration you should receive an email confirming your selections.  If you do not receive an email, please contact the WAFCA office (608-257-5939).

To submit registrations for multiple attendees, please e-mail Sharon Gust directly and list the workshop dates desired--include names, email addresses and phone numbers for all attendees.

If your agency does not subscribe to SWIP, the registration fee is $95.00 per workshop.  We must receive your $95.00 check within three business days of submitting this registration.   Payment must be received in order to confirm your registration.  Please make your check payable to WAFCA and mail to:  WAFCA, 131 W Wilson Street, Suite 901, Madison, WI  53703.

To be placed on a waiting list for a full workshop, e-mail Sharon Gust directly with the requested date.  It is recommended you furnish a cell phone number to contact you.  Also furnish all the information asked for on the regular registration form.
