SWIP Registration

Register for any SWIP Training Workshop with this form. Within two weeks of submitting this registration you should receive an email confirming your selections. If you do not receive an email, please contact the WAFCA office (608-257-5939).

To submit registrations for multiple attendees, please e-mail Sharon Gust.

If your agency does not subscribe to SWIP, the registration fee is $90.00 per workshop. Payment must be received at least two weeks prior to the workshop to reserve your place. Please make your check payable to WAFCA and mail to: WAFCA, 131 W Wilson Street, Suite 901, Madison, WI 53703
